Bon Securs Health System is hiring a life coach for their Norfolk, VA office.

Position Summary

Under the general supervision of the Director of Care Management will work independently and as part of a team to provide support, health education, and case management services to patients and their families using an empowerment approach in coordination with health care providers and systems.

Job Requirements

Minimum Requirements

1. Minimum of high school diploma or GED. LPN preferred. Bilingual preferred,
2. Knowledge and understanding of low-income and minority communities in Norfolk.
3. Ability to operate a variety of standard office machines and equipment. Mastery of basic computer skills. Two to
three years’ experience in human services, case management and/or work with families. Experience working
with healthcare systems and community-based organizations. Experience working with multicultural
communities in a culturally competent manner.

Preferred Requirements

1. License Practical Nurse or other Medical Qualifications.
2. Must have a valid Virginia driver’s license.
3. Ability to work as part of a multidisciplinary team.
4. A commitment to serving vulnerable populations.
5. Strong organizational skills with excellent written and oral communication skills.
6. Skilled in gathering and reporting information, as well as record keeping. Ability to work effectively with staff,
community members, family members, and external agencies.
7. Demonstrated problem solving and decision-making skills with ability to exercise initiative.
