This listing is no longer available. _______________________________________ The Health Coach for the Life in Balance Program provides professional and confidential face to face, individual and group coaching support to program participants while assisting them in establishing and meeting certain health and wellness goals. Demonstrates an ability to deliver effective coaching in a manner that promotes behavior change. Appropriately adapts coaching and education methods based on our integrative programs objectives to accommodate the unique physical, psychosocial, cultural, and age-specific needs of each participant served. Treats all participants with courtesy, dignity, respect and professionalism. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the patient served. Must demonstrate knowledge of the principles of growth and development as it relates to the different life cycles.
Also, this position has a flexible schedule as the hours range.
Minimum Qualifications
- Education:
Master’s or Bachelor’s degree in nutrition, health promotion, health education, community health, public health, exercise physiology, kinesiology or behavioral health related fields; Will consider broad and applied knowledge of behavioral change, chronic care management, health promotion and education for unrelated degrees. In lieu of a degree, will accept four (4) years of practical experience in behavioral change, chronic care management, health promotion and education in addition to qualifications listed below. - Experience:
Two (2) years experience as a Wellness Coordinator, Health Coach, Health Educator or other health care-related experience, which includes individual or group facilitation in health behavior change, chronic disease management, or weight and nutritional counseling. - Licenses/Certification:
Certified as a Chronic Care Professional (CCP) Health Coach or Registered Health Coach (RHC) or Certified Health Education Specialist (CHES) or Master Certified MNTR JJ
