    4 hours per week/Flexible schedule

    As part of the wellness program at CORT Furniture, our goal is to have healthy employees and run the business at top efficiency. In addition to encouraging healthy habits for our employees the Health Coach is responsible for helping to decrease employee absenteeism, health care costs, and work related injuries – while increasing employee morale, physical fitness and productivity.
    We are currently seeking a Health and Fitness Coach to join our team on-site in Paramount. The Health care coach works with employees one-on-one or in small groups to assist in meeting health and wellness goals related to weight, nutrition, exercise, stress, smoking cessation, work/life balance and general health.
    Essential Functions:
    • Assists in setting health and wellness goals through individual and/or group sessions.
    • Assists with the collection of biometric data such as height, weight, and demographics, related to measurement of employee goals.
    • Coordinates and conducts wellness programs in areas such as nutrition, stress management, weight management, smoking cessation, and exercise.
    • Coach employees in-person and by phone.
    • Collaborates with health center staff to coordinate and promote health and wellness programs
    • Works closely with third party health and wellness vendors.
    • Promotes a culture of health and wellness through ongoing participation in client worksite meetings, task force projects, and client-directed outreach programs.
    • Assists the Operation Manager with assessment and maintenance of reports and evaluation of the coaching program.
    • Participates in ongoing communication with the local clinic.
    • Interacts directly with the client or client representative(s).
    • Assists in training of other associates as directed and completes other duties as assigned.
    • This job has no supervisory responsibilities

    JOB REQUIREMENTS

    Required Qualifications:
    • Bachelor’s degree in Exercise Science, Kinesiology, Health Promotion or equivalent work experience
    • Minimum 1 year experience working in a health and fitness industry
    • Intermediate skill with the Internet & applicable software, Spreadsheet and Word Processing software particularly PowerPoint
    • Strong verbal and written communication and presentation skills in the English and Spanish language

    Preferred Qualifications:
    • National Recognized Personal Training Certification
    • Masters in Exercise Science, Health Promotion & Education, social work, counseling or a health related discipline OR equivalent experience working as a counselor, social worker, dietician or fitness professional
    • Minimum 1 year experience working as a wellness coach
    • Additional certifications in wellness/fitness specialties are desirable Experience in health promotion or work site wellness and public speaking

    Other Qualifications:
    • Requirements as set forth in the specific contract for the site may include working flexible hours including evenings, nights and weekends.
    • Travel for training may occasionally be required.

    • Excellent written and verbal communication
    • Experience with computer and web-based applications to manage consumer data
    • Ability to work non-traditional and flexible work hours as needed to promote a positive participant experience. This may include early mornings, late evenings, and weekends.

    Additional Desired Qualifications:
    • Direct experience with the Diabetes Prevention Program
    • Experience with coaching techniques, motivational interviewing, and other behavior modification strategies that encourage activation and behavior change
    • Registered Dietitian, Certified Health Education Specialist (CHES), or other related wellness certifications or qualifications
    • Prior experience in public health, health assessment tools, program planning, reporting, and evaluation

    The Client Relationship Manager-Health Coach will act as first level liaison between onsite employer leadership teams and USPM; communicate with USPM contract manager, implementation, and health services. Assist in the development, implementation, achievement and periodic evaluation of the onsite wellness campaigns goals and objectives related to Prevention Plan population risks, coaching, care collaboration, productivity and overall goal achievement. Conduct analysis, assessments, coach on action and prevention plans, planning, and collaboration for lifestyle changes and behavior modification support and participate in formal presentations of health program progress to clients. Coordinate and collaborate with appropriate services to enhance improvement of care needs; assist in the achievement and maintenance of program and Company goals and objectives, outlined in the Program design and according to the Client Contract terms.

    Desired Skills & Experience

    Bachelor’s Degree in health education, health promotion or public health required; or Registered Nurse; or Licensed Practical Nurse. The ideal candidate will have prior experience with wellness, individual health coaching, corporate wellness program design and/or implementation. Professional memberships and/or certifications related to health coaching or worksite wellness preferred (e.g. NWI, ACSM, WELCOA).

    Our JOB COACHES are responsible for providing initial training and then intermittent monitoring of a developmentally disabled (MR/DD) worker at a community job site until the worker is able work there on an independent basis. The duties include teaching job set up procedures and self supplying of materials to MR/DD workers, providing feedback to Employment Specialists regarding the worker’s vocational performance and transporting the individuals to and from work sites with a company vehicle.
    Requirements
    The ideal candidate will possess at least a high school diploma and must have a valid New York State driver’s license. Previous experience working with developmentally disabled population preferred. Excellent oral and written skills, the ability to interact effectively with coworkers, consumers, families and outside contacts and,the availability and willingness to work in varied environments must!! Candidates must be able to work evening morning and evenings.
