    Job Description:

    This position is responsible for conducting telephonic health coaching sessions based on Provant’s risk stratification methodology. The role of the health coach is to support and clinically educate health coaching clients as they migrate to lower risk stratifications. The telephonic health coach is required to maintain clinical progress notes for each coaching session and will be required to maintain their appointment schedule.

    Onsite Health Education Coordination
    Health Education Program Development
    Clinical Research
    Client Specific Training
    Clinical Training
    Other Projects Assigned by Management
    Portal Questions

    Preferred Qualifications:

    Wellness Coaching certifications, knowledge of specific disease and lifestyle related topics such as weight management, smoking cessation, chronic conditions and stress management. Experience with motivational interviewing or counseling skills

    Special Work Requirements:

    Flexibility and ability to extend work time beyond standard work hours as dictated by the needs of the company.

    Desired Skills and Experience

    Experience and Education requirements: (examples may include some or all of the following)

    Computer systems
    Writing skills
    Registered Dietitian, Registered Nurse, Exercise Physiologist, Health Educator REQUIRED (minimum of a 4 year degree in health related field)
    HIPAA compliance
    Continuous Improvement/Customer Focus: Ability to contribute to the company as it continues to grow; ongoing re-evaluation of existing processes; professional interactions with clients and partners; consistently suggesting of solutions along with discovered issues; a firm understanding that the goal is always outstanding customer satisfaction.
